What is joint working
In Mosaic, ‘joint working’ allows workers from different teams to be involved in the same case activity e.g. an assessment or review completed by an LPFT and an LCC SW or CCO together.
The next action Adult Referral for Joint Involvement is used to request involvement from LPFT or LCC Social Work. This is a 'send immediately' action available from most Adult Care workflow steps and should be assigned to the relevant LPFT or Adult Care Team.
The Adult Additional assessor input required request, sent from the Requests toolbar icon, can then be used to invite the LPFT/LCC worker into the workflow step to add information.
To transfer a case to LPFT, LCC workers should complete the Adult Mental Health Social Care LPFT Referral step. A next action of Adult Mental Health Social Care LPFT Triage should then be assigned to Adult Mental Health Social Care LPFT Referrals using 'Pass to Worker'. The referral must be finished using the green and white finish tick to ensure the Triage step is sent to LPFT.
