Assessment versus reviews

Generally, you should complete a new assessment if there is a change in need or if the person requires a new service type to meet their needs.

The only exception is if you have previously assessed for a temporary admission (STC) with a view to placing someone in permanent residency (LTC), in which case you can arrange the permanent residency service from a review. If you are unsure, please speak to your manager.

To arrange new services, you will need to complete a Needs Assessment, a Care and Support Plan (including Support Plan Costings), and a Purchase Service Request.

You will also need to ensure a Review of Care and Support Plan is scheduled and assigned to yourself or the appropriate team. Reviews are required for all cases receiving services.

For open cases, you will need to add the Needs Assessment as a next action from the Review of Care and Support Plan already scheduled on the record.

It is important to note, small permanent or temporary changes to existing services can be made from a Review of Care and Support Plan:

  • Small permanent changes = complete the Support Plan Costings (in the Review step) and progress to the Purchase Service Request. These steps should include details for the whole package of care and not just the details of the change you are requesting.
  • Temporary changes = complete the Change in Care Package Delivery Request. You should include the full details of the service that is changing.
