Ending services
The Change in Care Package Delivery Request is not to be used for ending services.
To request services are ended without fully closing a case:
If completing a review - update the Support Plan Costings form and add a next action of End Service Request, or Purchase Service Request if you are also increasing/decreasing other services at the same time.
If completing a plan - update the Support Plan Costings form and add a next action of Purchase Service Request.
To request services are ended when a case needs to fully close:
Complete the Proposed Case Closure and Business Support will request all services are ended as part of the case closure process.
