Ending my involvement or closing cases
If you have finished your case activity, but the person:
- requires a future review
- is receiving services
- has involvements from another team or worker
You will only need to end your worker relationship and not propose a case closure.
You will need to end your worker relationship and complete a Proposed Case Closure if the person:
- has died
- has moved out of county
- has declined all services
- is self-funding and doesn’t require any further involvement
- has no on-going services and no on-going worker or team involvements
