Internal Contacts referring to OT or SW
When an Adult Care Social Worker or Community Care Officer identifies the need to refer to Occupational Therapy or vice versa, an Internal Contact should be completed.
The Internal Contact should be assigned to self for completion and the appropriate next action should be assigned to the relevant team:
- To refer to SW select Adult Conversation Record
- To refer to OT select Adult OT Conversation Record
If the case is already open to OT or SW the Contact (Additional Information) should be completed instead to notify the team that further involvement is required.
