The Lincolnshire EHC Hub is an innovative digital platform for families, professionals and education settings. The platform engages with settings to contribute and collaborate on Education Health Care (EHC) needs assessments, plans and reviews.
The Hub provides a secure, shared area for everyone involved with a child or young person to make their contribution, see the contributions of others and all work towards a positive outcome. The Hub also provides the opportunity to access information at any point in the process and at any time of the day.
This approach will promote the voice of the child being at the heart of EHC needs assessments and plans, and provides a true narrative for the understanding of a child or young person’s needs and aspirations.
To submit an EHC needs assessment request, you won't need log in details, just click the "request needs assessment" on the login screen and follow the instructions to access the form. If you are unable to use the hub and would like to submit a request, please contact SEND business support by email, or phone 01522 553332.
Following the launch in February 2023 for EHC needs assessments, annual reviews of EHC plans are also now being facilitated by the Hub. In preparation for this, the local authority have been importing existing EHC plans to the system. This process is still underway – if you have a child or young person with an EHC plan and we have your email address, you will receive an email inviting you to set up an account in due course.
