Inappropriate Hospital Discharges
If a provider suspects there has been an inappropriate discharge from a hospital setting into their service; no harm has occurred and does not meet safeguarding concerns, the form below should be submitted for investigation.
Some examples of inappropriate discharge include:
- a service user returning to a residential home or their own home with no prior notice given to the Provider
- a service user being discharged without appropriate moving and handling equipment in place
- a service user being discharged without adequate medication supply
If you are unsure whether a form should be completed please contact your designated Contract Officer to discuss further.
It is important that all instances are investigated so that we can establish the cause of the issue and prevent reoccurrence. Please ensure all staff are aware of the process. All issues should be reported even if they were resolved following discharge using the inappropriate discharge form which is available to staff on Our intranet.
